How to become a Surrogate

To work with us you will need to make following steps:

1. Fill the questionnaire of a standard pattern.
2. Prepare your photos, photos of your children and a certificate of their health issued by therapeutist.
3. Prepare your passport, certificates of birth of your children, certificate of marriage and copy of passport of your husband (if you married), or certificate of dissolution of marriage, copy of your labour book.
4. Get a certificate from psychoneurologic dispensary at the place of your residence, fluorography test and therapeutist’s conclusion of your health state and absence of contraindications for pregnancy.
5. Specify the date of beginning of your cycle, its duration.
6. Undergo medical tests in accordance with the requirements of our partner clinics, but it is necessary to undergo the following three tests beforehand - HIV (AIDS) test, test on RW (syphilis) and hepatitis В and С, determine your blood type and Rh.
7. Undergo repeated examination of therapeutist, gynecologist and psychologist to determine if you suit this programme.

You will need to come to Kyiv with the purpose to undergo medical examination from the 5^th to 7^th or from 17^th to 21^st day of your cycle. There is an obligatory repeated arrival for undergoing additional tests from 2^nd to 5^th day of your cycle.
